The Baxter FDA warning letter of 2026 is a CDER warning letter dated September 29, 2026 that cites Baxter Healthcare Corporation's Round Lake, Illinois sterile injectables facility (FEI 1416980) for drug CGMP violations: cephalosporin cross-contamination, unvalidated sterile processing, weak visual inspection for particles, and inadequate investigations. It followed a March 23 to April 2, 2026 inspection that ended with a nine-observation Form 483.
FDA posted the letter on October 20, 2026, a week after Olympus disclosed its own multi-site device letter. This article sets the letter against the inspection record in Atlas (release of August 15, 2026, checked October 26, 2026): Round Lake's 38 FDA inspections, its four Form 483s since 2023, and the 483s written at other Baxter facilities in 2025 and 2026.
Key facts:
- FDA's letter is reference 320-26-132, issued by CDER's Office of Manufacturing Quality and addressed to Baxter International CEO Andrew Hider.
- The letter cites four violations under 21 CFR 211.42(c), 211.113(b) with 211.42(c)(10), 211.160(b) and 211.192.
- Baxter's environmental monitoring found cephalosporin on the non-beta-lactam side of the Round Lake building in several instances since January 2025, according to the letter.
- Round Lake has 38 FDA inspections on record in Atlas; 13 ended with a Form 483, four of them since August 2023.
- Across 47 Baxter facility records in Atlas, 102 of 244 FDA inspections ended with a Form 483.
- Baxter had 15 business days from receipt to respond in writing.

What does the 2026 Baxter warning letter cite?
The 2026 Baxter warning letter cites four CGMP violations at the Round Lake facility, all under 21 CFR Part 211, the drug manufacturing rules. FDA says the drugs made there are adulterated under section 501(a)(2)(B) of the FD&C Act because the methods, facilities and controls do not meet CGMP. This is a drug letter from CDER, not a device letter.
The full letter is on FDA's site. Many figures are redacted, but the structure is clear:
| # | Section cited | What FDA found at Round Lake |
|---|---|---|
| 1 | 21 CFR 211.42(c) | Cephalosporin and non-beta-lactam products made in one building separated by a wall and dedicated HVAC units; cephalosporin detected on the non-beta-lactam side; failing environmental samples discarded |
| 2 | 21 CFR 211.113(b), 211.42(c)(10) | Biological indicator survivors during sterilization requalification runs; media fills that simulated interventions far less often than production; missing or weak airflow studies |
| 3 | 21 CFR 211.160(b) | Manual visual inspection unable to reliably find particles; a returned cefazolin unit held a cardboard particle (May 9, 2025) |
| 4 | 21 CFR 211.192 | Rising container leak rates while nonconformance reports fell; a 2025 cefazolin sterility failure invalidated without a full investigation and the lot released |
On the first violation, FDA restates its long-held position that "no safe level of beta-lactam contamination" has been shown to be a tolerable risk. On the second, it calls Round Lake's equipment, process and airflow design an "excessive contamination risk." The letter also says FDA held a regulatory meeting with Baxter on January 15, 2025 about similar violations at the facility, and that FDA has cited similar problems at other Baxter facilities, which it reads as a failure of management oversight.
FDA asks Baxter to hire a consultant qualified under 21 CFR 211.34 for a six-system audit, to reassess the physical separation of cephalosporin work, to revalidate sterilization and repeat media fills after remediation, and to review every lot in distribution for visible particles. Until the violations are corrected, FDA may withhold approval of applications that list Round Lake and may withhold export certificates.
What inspection led to the Baxter warning letter?
The inspection behind the Baxter warning letter ran from March 23 to April 2, 2026 at Round Lake and ended with a Form 483 dated April 2, 2026. The April 2, 2026 Form 483 carries nine observations and four investigators, led by Robert J Ham, who has 175 inspections and 319 Form 483 observations on record in Atlas.
Baxter answered the 483 on April 23, 2026, inside the 15-business-day window, and sent later updates. FDA's letter reviews that response violation by violation and finds it inadequate each time: no plan for complete separation of cephalosporin work, no commitment to replace equipment, and a retrospective review of leaking containers limited to one year. In the meantime, on July 17, 2026, Baxter agreed to recall cefazolin lot LD175708 after FDA contacted the company about the cardboard particle.
Here is how the April 483 lines up with the letter:
| Section | April 2, 2026 Form 483 | September 29, 2026 warning letter |
|---|---|---|
| 211.42(c) separate defined areas | Yes | Yes |
| 211.113(b) sterile process validation | Yes | Yes |
| 211.192 investigations | Yes | Yes |
| 211.160 laboratory controls | 211.160(a) | 211.160(b) |
| 211.42(c)(10) aseptic processing areas | No | Yes |
| 211.100(b), 211.165(d), 211.25(a), 211.63, 211.67(b) | Yes | No |
Three sections carry straight from the 483 into the letter. That is the usual pattern: the warning letter keeps the observations FDA considers systemic and drops the ones a response could plausibly close. Our guide to the 483 versus warning letter response window covers how that decision is made.
Did Round Lake have earlier Form 483s for the same problems?
Round Lake had earlier Form 483s, but the public record shows only one earlier citation of a section that reappears in the letter. The Round Lake facility record lists 38 FDA inspections from 1999 to 2026, 13 ending with a Form 483 and 57 public observations.
The repeat is laboratory controls. A biologics inspection from November 17 to 25, 2025 ended with a five-observation Form 483 citing 211.160(b), alongside 211.22(d) quality unit procedures, 211.25(a) training, 211.68(b) computer controls and 211.194(a)(1) laboratory records. Five months later the drug inspection cited 211.160(a), and the letter cites 211.160(b) for visual inspection.
The rest of the history is mostly devices. Round Lake's 483s from 2011 to 2015 cite 21 CFR 820.100(a) corrective and preventive action ten times and 820.72(a) equipment eight times, and the August 2023 form cites 820.30(g) design validation. Two earlier drug inspections matter too: one from June 30 to July 7, 2015 is recorded with a warning letter outcome, and one from July 23 to August 1, 2024 ended with a 483 whose observation text is not public. The letter's reference to a January 2025 regulatory meeting suggests FDA had already raised contamination control before the 2026 inspection.

| Guayama, Puerto Rico | 2650090 | 13 | 5 | Mar 2025 | 483 |
| Ahmedabad, India | 3004610460 | 13 | 11 | Jun 2025 | 483 |
| Medina, New York | 1314492 | 12 | 6 | Apr 2025 | 483 |
| Marion, North Carolina | 1025114 | 12 | 9 | Feb 2024 | 483 |
| Aibonito, Puerto Rico | 2649614 | 11 | 7 | Nov 2023 | 483 |
Jayuya and Ahmedabad have the highest 483 rates, at 10 of 14 and 11 of 13. Guayama and Ahmedabad each have an earlier inspection recorded with a warning letter outcome, in 2015 and 2017. Every facility here can be searched in the FDA Form 483 database.
Which Baxter Form 483s were issued in 2025 and 2026?
Baxter received six Form 483s with public observation detail from January 2025 to April 2026, at five facilities, according to the Atlas Form 483 directory (release of August 15, 2026). A seventh, from Ahmedabad in June 2025, is on record without observation text.
| Facility | Form 483 date | Program | Observations | Sections cited |
|---|---|---|---|---|
| Alliston, Ontario | Jan 31, 2025 | Human drugs | 4 | 211.110(a), 211.192, 211.80(b), 211.84(d)(2) |
| Guayama, Puerto Rico | Mar 26, 2025 | Human drugs | 1 | 211.22(d) |
| Medina, New York | Apr 4, 2025 | Devices | 2 | 820.100(a), 820.198(a) |
| Cleveland, Mississippi | Sep 29, 2025 | Human drugs | 5 | 211.160(b), 211.192, 211.58, 211.67(a), 211.68(b) |
| Round Lake, Illinois | Nov 25, 2025 | Biologics | 5 | 211.160(b), 211.194(a)(1), 211.22(d), 211.25(a), 211.68(b) |
| Round Lake, Illinois | Apr 2, 2026 | Human drugs | 9 | 211.42(c), 211.113(b), 211.192, 211.160(a), 211.165(d), 211.100(b), 211.25(a), 211.63, 211.67(b) |
This table supports FDA's network comment. 21 CFR 211.192 appears at Alliston, Cleveland and Round Lake within 15 months. 211.160(b) appears at Cleveland in September 2025 and Round Lake two months later. 211.22(d) appears at Guayama and Round Lake. For scale, across 90 recent human-drug 483s in the Atlas playbook sample (October 2026), 211.192 appears on 31%, 211.160 on 32%, 211.113 on 23% and 211.42 on 14%. Baxter's citations are common ones; the repetition across sites is what FDA treats as a corporate signal.
How does the Baxter letter compare with the Olympus and Fresenius letters?
The Baxter letter is a single-facility drug letter, while Olympus received one device letter covering six facilities. The closest match is the September 22, 2026 letter to Fresenius Kabi's Melrose Park facility, another Illinois sterile injectables site cited under 211.192 and 211.113(b) for aseptic processing and visual inspection.
| Baxter, Round Lake | Fresenius Kabi, Melrose Park | Olympus | |
|---|---|---|---|
| Letter date | Sep 29, 2026 | Sep 22, 2026 | Oct 7, 2026 |
| FDA center | CDER (drugs) | CDER (drugs) | CDRH (devices) |
| Facilities | 1 | 1 | 6 |
| Lead theme | 211.42(c) separation | 211.192 investigations | CAPA, cited on all six 483s |
| Recall before the letter | One cefazolin lot, July 2026 | Three famotidine batches | None required |
Our write-ups of the Olympus warning letter and the Fresenius warning letters show the same thread: the letter arrives after a 483 response FDA found too narrow.
What has Baxter said about the warning letter?
Baxter had not issued a press release or SEC filing about the Round Lake warning letter as of October 26, 2026, based on our search of its investor releases and its 2026 quarterly reports. The company's public record on the issue so far is the July 2026 cefazolin recall and its April 23, 2026 response to the Form 483, which FDA describes in the letter.
The open questions are practical. Round Lake makes sterile injectables, including cefazolin, so hospital pharmacy and supply teams will watch whether remediation affects supply; FDA's letter tells Baxter to contact the Drug Shortages Staff before any action that could disrupt it. The letter will close only after FDA confirms the corrections, usually through re-inspection. To follow the posting and closeout of letters like this one, see our FDA warning letter database guide.
